  • Changes in form permissions

  • Movement of the form between accounts or groups

Note: These issues can affect not only PowerPoint but also other platforms where Forms are embedded (e.g., Teams, OneNote, web pages).

Resolution Steps

1. Try the Refresh Button

  1. Open the settings pane for the embedded form in PowerPoint.
  2. Click the Refresh button to reload the form.
  3. This may resolve the issue without needing to reinsert the form.

2. Reinsert the Form

If refreshing does not work:

  1. Delete the embedded form from the slide.
  2. Reinsert it using the Microsoft Forms integration in PowerPoint.
  3. This ensures the embed uses the most current and active link.
