Overview
Generally, changes to your coverage are not permitted outside your initial new hire enrollment period or annual open enrollment. However, if you experience a qualifying status change you are given a 30 day window to make applicable changes to your benefits elections. Qualifying status changes include, but are not limited to:
- marriage or divorce
- birth, adoption, or death
- gain of other group health coverage
- loss of other group health coverage
- change from full-time to part time employment for you or your spouse
- change in coverage under other employer's cafeteria plan or qualified benefits plan
Should you experience a status change throughout the year, you must process benefit changes within thirty (30) days or you must wait until the next open enrollment period. Coverage changes made mid-year, including applicable premiums, will be effective on the date of the change in status (ie. loss or gain of coverage).
Process
To make changes, you must do the following within thirty (30) calendar days of the status change:
- Make changes on the ICUBA benefits website
- You can also access the ICUBA site from rollins.okta.com (ICUBA by bswift)
- Once logged in, go to Life Events and select the appropriate life event

- Note that you are not able to process a life event earlier than 2 weeks prior to the event date.
- Upload documentation of your status change on the ICUBA benefits website and be sure documentation includes all of the following:
- Proof of event (marriage license, birth certificate, divorce decree, COBRA notice if losing coverage, benefit confirmation if gaining coverage)
- Effective dates (date of divorce, last day of prior coverage, first day of new coverage, date of marriage)
- Name of employee or dependent affected
- How to upload documentation:
- Go to rollins.okta.com, select ICUBA by bswift

- Go to My Profile, then Employee File and upload your document

- Note: Covered retirees should contact Ameriflex at 866-377-5102 to process life event
